Final Shutdown Notice

 

Thank you for allowing us to serve as your dermatology clinic for the past 12 years. We are closing the clinic and terminating all provider-patient relationships. We are available for medical services, including emergency access services, for the next 45 days. Our final shutdown date will be Friday, July 28, 2023. If you need something from us, please contact us before that date.

Follow-up Appointments. At this time, we have plenty of availability for follow-up appointments. If you need an appointment, you can schedule it in the usual way, including online.

Refills. If asked, we will refill your medicine to help tide you over until you can find a new dermatology clinic. An appointment may be required in order to authorize refills. The best way to request refills is to contact your pharmacy who will then send us a request, which we will then authorize. 

Balance due. Please settle your balance prior to the shut down date. You may settle your balance in the usual way (check, online payment at our website, pay by credit card over the phone).  After the shutdown date, we will still be working old claims and sending out bills. 

Medical records. The easiest way to secure your medical records is to first choose a new dermatology clinic, then let us know so we can transfer your records.   

Phone lines. Our phone lines will be open for routine services until the final date. After that, you will be able to leave a voicemail for records requests.

Choosing a new dermatology clinic. There are a number of area dermatology clinics who are still accepting new patients. We have provided a list below.

 

Support Staff. The team has worked tirelessly to ensure your experience was a good one. I’m very grateful to have employed over forty employees, both past and present. 

Final thoughts. It has been an honor to serve as your dermatology provider. I am proud of the thousands of patients we have helped over the years. I wish you all the best of luck in all of your journeys.
